Advance Care Directives
Advance Care Directive is about planning ahead for your future healthcare needs. It is also about letting your family and doctors know your decisions.
There are four simple steps:
- Appoint someone to be your ‘Support Person’
- Appoint a "Medical Treatment Decision Maker" (previously a Medical Power of Attorney)
- Talk with your family, friends and doctor about your values 'values directives' and health care wishes 'instructional directive'
- Write down your wishes for future treatments using an 'advance care directive' with your GP
