Omeo District Health (ODH) provides Home Based Services to help eligible people remain living safely, comfortably, and independently in their own homes.
We understand that, as we get older or experience changes in our health, everyday activities can become more difficult. You may need a little help with household tasks, personal care, preparing meals, getting to appointments, or staying connected with your community.
Getting support doesn't mean losing your independence.
The right assistance can help you maintain your independence, wellbeing, and quality of life for longer.
Omeo District Health delivers services through several government-funded programs, including:
Commonwealth Home Support Programme (CHSP)
Home and Community Care Program for Younger People (HACC PYP)
Support at Home (SAH)
Post Acute Care (PAC)
The program you may be eligible for depends on your age, circumstances, assessed needs and the services available to you.
The Commonwealth Home Support Programme provides entry-level support for older people who need a little extra help to continue living independently and safely at home.
The program focuses on helping you maintain your independence by supporting you to do the things you can, rather than taking over tasks unnecessarily.
Who may be eligible?
You may be eligible if you are:
65 years or older;
an Aboriginal or Torres Strait Islander person aged 50 years or older; or
aged 50 years or older and homeless or at risk of homelessness.
An aged care assessment is required to determine your eligibility and the services you may receive.

Who may be eligible?
HACC PYP supports:
Victorians from birth to 65 years of age; and
Aboriginal people from birth to 50 years of age.

The program is designed to meet assessed care needs, whether you need help with everyday activities or more complex support to maintain your health and independence.
Who may be eligible?
Support at Home may be available to eligible older people aged 65 years and over, with certain exceptions for Aboriginal and Torres Strait Islander people and people who are homeless or at risk of homelessness.

Support at Home short-term pathways
In addition to ongoing services, Support at Home includes three short-term pathways designed to address specific needs.
Restorative Care Pathway
Provides time-limited, multidisciplinary support to help you improve or regain your ability to undertake everyday activities and maintain your independence.
Assistive Technology and Home Modifications (AT-HM) Scheme
Helps eligible participants access assistive technology, equipment and home modifications to improve safety, accessibility, and independence at home.
End-of-Life Pathway
Provides eligible participants with additional support to remain at home at the end of life, where this is their preference and their needs can be safely supported.
Access to these pathways depends on eligibility and assessment.
The Post Acute Care (PAC) Program provides short-term support to people returning home after a stay at a public hospital or a visit to a hospital emergency department.
The program helps people recover safely at home, regain their independence, and reduce the risk of an unplanned return to hospital.
Support is tailored to each person's individual needs and is generally provided for a short period while they recuperate.
Who may be eligible?
PAC may be available to people who are being discharged from a public hospital and have been assessed as needing short-term, community-based support to recover safely at home.
A referral is generally arranged by the hospital team as part of discharge planning.
What support can be provided?
Depending on the person's assessed needs and the services approved, support may include:
Community nursing
Personal care, including showering and dressing
Assistance with everyday activities at home
Home care to support recovery and independence
How do I access Post Acute Care?
If you are preparing to leave hospital and feel you may need additional help at home, speak with your hospital nurse, treating team or discharge planner. They can assess your needs and discuss whether a PAC referral is appropriate.
PAC is intended to provide short-term assistance during recovery. If you require ongoing support after your PAC services finish, your care team can help explore longer-term options.
