Home Based Services

The Home Based Services program is aimed at assisting frail aged people, people with disabilities and their carers to remain living independently at home in a community setting. 

In August 2021 ODH became a Home Care Package Provider, with 2 clients signing up within the very first month. With word spreading fast and community members wanting more localised support services has meant our Home Based Services Team has been very busy.   

The Commonwealth Home Support Program provides a range of entry-level aged care services for older people who need assistance with daily tasks to continue living independently at home and in their community. Offering services such as:

• Domestic Assistance

• Personal Care

• Respite Care

• Home Maintenance / Home Modification

• Meals on Wheels and assistance with meal preperation

• Social Support Group SSG (formerly, Planned Activity Group / PAG)

• Home Based Nursing 

In order to support these services, Omeo District Health provides independent assessment for clients through the Regional Assessment Service (RAS).

To be able to access our home care program you will need to submit an application to My Aged Care.
